REPORTS TO: WA Coordinator
POSITION PURPOSE:
The BA level WA Facilitator serves as the contact point for children and families in the WA program, as well as with service providers and others in the community. The Facilitator will ensure that the principles of Wraparound and the Core Practice Model are adhered to by all team members and that all the child/family strengths, needs and underlying needs are identified.
QUALIFICATIONS:
BA or BS in Human Services, Social Work or Psychology or three years’ experience working with high risk children/youth in either child welfare, probation or mental health systems in Wraparound. Demonstrated organizational skills. Comfort with WA approach and expectations. Must maintain a valid California driver’s license, insurance and driving record which complies with agency’s insurance carrier standards
SPECIAL SKILLS:
Ability to act as leader of individual WA Child & Family Team and ensure continuity of care. Ensure that all identified services are provided in a timely and appropriate manner. Ensure application of family-centered practice by the entire team. Ensure continuous input and feedback from the family and service providers. Provide individual rehab and linkage services as needed. Meet established documentation and productivity expectations. Be available and responsive by agency cell phone for crisis management 24/7.
